#18784 - 01/28/10 05:51 PM Changing The Date for Problems?

Can you change the date on a problem in the problem list? We would like this to reflect the date the problem actually became an issue for the patient (not just when we added it to the system). Is there any way to change the date and set it sometime in the past?

If not, how are people documenting the date-of-onset for problems in the problem list? Also, how are people dealing with issues that don't necessarily have an ICD-9, but would like to keep track of as a problem?

Thanks,

S.K.

To have the date reflect an older date you would change the date for the encounter. This will carry the date over to the summary sheet in the problems list.

Some practices that want to keep track of problems that don't have ICD-9 codes create one so they can have it populate in the summary sheet. you can do this by going to the search ICD-9 button in the encounter section. You than click on edit ICD-9. Click on add and enter a code like 0001.

That would be a great way of entering the surgeries. Thanks for the suggestion.

But how can you bill that?
Inconsistently, (it isn't always a problem) when this comes up I write the problem in the box for medical history, with the date of onset. Then I copy and paste the ICD of the ridiculous Dx that I am forced to use for billing. (Blah-Blah not otherwise specified etc.) That allows me on future visits to glance to the left side of the chart and find the condition, and see immediately what I called it, and what code to pull up for the bill.
